Abstract:
- Le suivi thérapeutique du tacrolimus joue un rôle crucial dans le traitement des patients ayant subi une
transplantation rénale afin d’en garantir l’efficacité et la sécurité.
Le but de cette étude est de mettre en évidence l'importance du suivi thérapeutique du tacrolimus pour
ajuster la posologie dans diverses situations cliniques en transplantation rénale tout en appuyant sur
l’intéret de l’utilsation de l’AUC(0-12) pour une meilleur prise en charge des patients.
On a examiné 32 patients transplantés rénaux suivis au niveau du CHU Ben badis de Constantine, ainsi
que 10 patients suivis au niveau de l’HMRUC, à travers une étude déscriptive rétrospective menée
pendant 5 mois. L’étude s’est basée initialement sur le dosage du taux résiduel du tacrolimus (CHU : 50
prélèvements , HMRUC : 17 prélèvements) par une technique immunologique, secondairement sur
l’analyse des résultats de l’AUC(0-12) éstimés par méthode Bayésienne.
Les résultats de ce suivi ont montré que le STP du tacrolimus permet d’individualiser
l’adaptation de sa posologie et d’atteindre plus précocement les concentrations cibles. Ils ont
montré aussi une concordance imparfaite entre la C0 et l’AUC0-12 et que ce dernier est le meilleur
reflet de l’exposition réelle au tacrolimus que la C0. - Therapeutic monitoring of tacrolimus plays a crucial role in the treatment of patients who have
undergone kidney transplantation to ensure its efficacy and safety. The aim of this study is to highlight
the importance of therapeutic monitoring of tacrolimus for dose adjustment in various clinical situations
in kidney transplantation, while emphasizing the interest of using AUC(0-12) for better patient
management.
We examined 32 kidney transplant patients followed at CHU Ben Badis in Constantine, as well as 10
patients followed at HMRUC, through a descriptive retrospective study conducted over 5 months. The
study initially relied on the measurement of tacrolimus trough levels (CHU: 50 samples, HMRUC: 17
samples) by an immunological technique, and subsequently on the analysis of AUC(0-12) results
estimated by the Bayesian method.
The results of this monitoring showed that therapeutic monitoring of tacrolimus allows for
individualized dose adjustment and achieving target concentrations more quickly. They also showed an
imperfect concordance between C0 and AUC(0-12), with the latter being a better reflection of the actual
exposure to tacrolimus than C0.
