Prescription des Antibiotiques en Odontostomatologie Enquête Auprès des Médecins Dentistes De Constantine et de quelques wilayas de l’Est Algérien

Abstract

RESUMÉ : Introduction : Les antibiotiques sont des substances chimiques naturelles ou de synthèses utilisées pour traiter les infections bactériennes .Il existe plusieurs familles d'antibiotiques, chacune étant efficace contre une bactérie ou un groupe restreint de bactéries. La majorité des maladies bucco dentaires sont causées par le biofilm buccal qui peut être éliminé par différentes procédures (drainage, traitement radiculaire, et détartrage...) mais dans certaines situations l’infection aigue ; quel que soit le niveau du risque infectieux, la prescription d’antibiotiques est recommandée en complément d’un traitement dentaire actif. Objectif : - Le bon usage des antibiotiques en pratique dentaire - Déterminer les antibiotiques les plus prescrits - Déterminer les indications de l’antibiotique. Matériels et méthode : Il s'agit d'une enquête réalisée auprès de 264 médecins dentistes de Constantine et des autres wilayas de l'Est Algérien (Constantine, Skikda, Tebessa, Oum el Bouaghi, Mila, Batna, Khenchla) durant la période : Octobre 2021et Avril 2022. Résultats et discussion : - Les généralistes représentent 86,74% des médecins dentistes interrogés. -Les médecins dentistes prescrivent les antibiotiques chez les patients qui présentent un risque d’EI dans 85,98% des cas, ainsi que chez les Immunodéprimés dans 68,18 % des cas avec l’amoxicilline comme molécule de choix à raison de 2g ; 1 heure avant l’acte. - L’antibiotique le plus prescrit en association est l’Amoxicilline + Métronidazole dans 74 ,62% des cas. - Les critères de choix de l’antibiotique des médecins dentiste sont: la présence d’infection dans 93,93% des cas et un spectre d’activité large dans 62,5% des cas. - L’enquête a révélé que 70% des médecins dentistes font l’association des antibiotiques en 1ere intention, Et près de 86% des médecins dentistes confondent entre l’antibiothérapie et l’antibioprophylaxie. -Nous avons constaté également que les spécialistes prescrivent la monothérapie en 1ere intention et l’association en 2ème intention.ABSTRACT: Introduction: Antibiotics are natural or synthetic chemical substances used to treat bacterial infections. The majority of oral diseases are caused by the oral biofilm which can be eliminated by dif-ferent procedures (drainage, root treatment, and scaling...) but in certain situations the severe infection; whatever the level of infectious risk, the prescription of antibiotics is recommended in addition to active dental treatment. Objective: - The proper use of antibiotics in dental practice. - Define the most prescribed antibiotics. - Define the indications of the antibiotic. Materials and methods: This is a survey of 264 dentists from Constantine and other wilayas of eastern Algeria (Con-stantine, Skikda, Tebessa, Oum el Bouaghi , Mila , Batna , Khenchla ) during the period Octo-ber 2021 and April 2022 . Results and discussion: - General practitioners represent 86.74% of the dentists questioned. -Dentists prescribe antibiotics in patients who present a risk of IE in 85.98% of cases, as well as in immunocompromised patients in 68.18% of cases with amoxicillin as the molecule of choice at a rate of 2g; 1 hour before the act. - The most prescribed antibiotic in combination is Amoxicillin + Metronidazole 74.62% of cases. - The criteria for choosing the antibiotic for dentists are: the presence of infection in 93.93% of cases and a wide spectrum of activity in 62.5% of cases. -The survey revealed that 70% of dentists combine antibiotics in first intention, and nearly 86% of dentists confuse between antibiotic therapy and antibiotic prophylaxis. -We also found that specialists prescribe monotherapy in first intention and the association in 2nd intention.
