dc.description.abstractUn frein n'attire pas forcément l'attention lors d'un examen buccal de routine car de nombreux dentistes ne sont pas conscients des variations morphologiques du frein labial maxillaire et les interprètent à tort comme des entités pathologiques. Néanmoins, la détection d'un frein maxillaire anormal ou carrément absent peut représenter un indicateur très utile dans le diagnostic d'un large éventail de conditions syndromiques et non syndromiques. Un frein pathologique peut occasionner difficultés lors de l’allaitement, diastème médian, difficulté de brossage et récession gingivale, instabilité prothétique et sourire disgracieux. La gestion du frein labial maxillaire aberrant nécessite une prise en charge chirurgicale. La frénectomie est une technique chirurgicale simple et rapide qui permet d’éliminer les tractions des freins pathologiques par une excision du frein ainsi que de leur attachement à l’os sous-jacent. Plusieurs techniques chirurgicales ont été proposées dans la littérature. La technique classique demeure la plus utilisée pour sa facilité de réalisation et pour la fiabilité des résultats thérapeutiques. La frénectomie doit être pratiquée dans les limites de ces propres indications. Ces dernières peuvent être d’ordre orthodontique, parodontal ou prothétique. Des études prospectives et contrôlées doivent être réalisées afin de déterminer le moment idéal d’intervention.Abstract The frenum is a mucous membrane fold that attaches the lip and the cheek to the alveolar mucosa, the gingiva, and the underlying periosteum. It may jeopardize the gingival health when they are attached too closely to the gingival margin, either due to an interference in the plaque control or due to a muscle pull. In addition to this, the maxillary frenum may present aesthetic problems or compromise the orthodontic result in the midline diastema cases, thus causing a recurrence after the treatment. The management of such an aberrant frenum is accomplished by performing a frenectomy. Frenectomy is the complete removal of the frenum, including its attachment to the underlying bone, while frenotomy is the incision and the relocation of the frenal attachment Frenectomy can be accomplished either by the routine scalpel technique, electrosurgery or by using lasers. The conventional technique involves excision of the frenum by using a scalpel. However, it carries the routine risks of surgery like bleeding and patient compliance.en_US
