dc.description.abstractRésumé : PBMS touche essentiellement des femmes après la ménopause qui présentent des signes de somatisation, de dépression et anxiété, à des valeurs supérieures à celles de sujets témoins. Il s’agit d’une douleur chronique souvent intense à type de brûlure, débutant à la langue pour envahir toute la cavité buccale, augmentant en intensité dans la journée avec un retentissement émotionnel important. L’étiopathogénie reste incertaine, évoquant la participation des hormones sexuelles féminines et des facteurs neuropathiques par possible neuropathie sensorielle des petites fibres de la muqueuse buccale. La normalité de l’examen clinique et des examens complémentaires la différencie des stomatodynies secondaires. Les approches thérapeutiques consistent en l’utilisation de doses faibles de clonazépam de façon topique sans déglutition ou systémique. L’association avec des antidépresseurs tricycliques a donné des résultats variables. L’approche par thérapie cognitivo-comportementale a été tentée avec succès par certaines équipes. Abstract: PBMS involves mainly women after menopause presenting somatization, depression and anxiety at values higher than control subjects. This chronic pain is often an intense burning pain beginning in the tongue the entire oral cavity, increasing in intensity through the day with a high level of emotional after effects. The causes remain uncertain. The participation of female sexual hormones and neuropathic factors has been suggested possibly through a sensory neuropathy of small fibers of the oral mucosa. Normal clinical examinations and nonclinical tests differentiate primary from secondary stomatodynia. The management consists in using low doses of topical clonazepam without swallowing or systemic clonazepam. The association of this drug with tricyclic antidepressants has given variable results. A cognitive behavioural management has been successfully attempted.en_US
